The WH-347 Rev. January 2025 edition: what is on it

If you have been filling in a WH-347 template for years, it is worth checking which edition it is. This page lists what can be read directly off the Rev. January 2025 edition of Form WH-347. It does not cover the rulemaking behind the form or any history beyond what the form itself shows. For anything else, check the DOL WH-347 page, and always compare the form you file against the current DOL PDF.

How to identify the edition

The burden statement estimates an average of 55 minutes to complete.

Page 1 header

Three checkboxes sit at the top:

The header fields are: project name; project no. or contract no.; certified payroll no.; prime contractor's/subcontractor's business name; project location; wage determination no.; week ending date; and prime contractor's/subcontractor's business address.

Page 2: six numbered items

Page 2 carries the project header, the certifying official's name and title, and the statement "I paid or supervised the payment of the laborers or mechanics..." followed by six numbered items:

  1. Payroll correct and complete; rates paid not less than the wage determination.
  2. Payrolls and basic records complete, accurate and available.
  3. Classifications reported are the work actually performed.
  4. Apprentices registered with OA or a recognized SAA, with an apprenticeship table (program name, OA/SAA, registered name of labor classification).
  5. Fringe benefits paid in cash and/or to bona fide plans, with an hourly credit for fringe benefits table (up to six plans per worker: name, type, plan number, funded/unfunded, hourly credit, and a total).
  6. Full weekly wages paid, with no rebates or deductions other than those permitted under 29 CFR part 3.

Then Additional Remarks and a signature block with signature, date, telephone number and email address, plus the falsification warning and a FOIA disclosure note. More on each item in the Statement of Compliance guide.

The 4(a)/4(b) restructure

Older editions of the WH-347 had a fringe item 4 with (a) for amounts paid to approved plans, funds or programs and (b) for amounts paid in cash. The Rev. January 2025 form has no items labelled 4(a) or 4(b). Item 4 covers apprentices, and fringe is handled by item 5, its hourly-credit table, and columns 6B and 6C. See the fringe benefit columns guide.

Our tool and the current edition

The WH-347 Layout Tool produces an optional layout that follows the Rev. January 2025 field and column list above, from a CSV or Excel export read in your browser. It does not fill the apprenticeship or hourly-credit tables and leaves the signature, date, phone and email blank. It is not affiliated with or endorsed by the U.S. Department of Labor, so check what you file against the current DOL PDF.
